Assessments and Reports with Create A Sense of Place

We can offer a variety of assessments, undertaken by appropriately trained and qualified Team Members such as Social Workers, Psychologists and Counsellors.

Our Assessments Include

  • Functional Behaviour Assessment
  • Safeguarding Assessment
    This is an in-depth assessment which takes a person on a journey to express their needs, wants and desires, and identifies any risks in their environment that may be causing them harm. This assessment also looks at life transitions and forward planning and develops strategies to help a person feel more prepared. This assessment can take between 8 and 12 hours.
  • Mental Health Assessments
    Identifying and responding to violent and aggressive behaviour. This assessment is to support people that may be at risk to themselves or others, involving a consideration of risk factors such as vulnerability, mental illness, situational triggers and environmental factors. This assessment can safeguard the person and provide support strategies and recommendations to workers. This assessment can take between 1 and 2 hours. To complete a report with recommendations and implementation, this will be an additional 2 to 4 hours.
  • Assessment of Sexual Knowledge
    This in depth assessment takes a person through questions with pictures to gain an understanding into their level of sexual knowledge. A detailed report is written which outlines recommendations and areas for capacity building and identifies risks to abuse and exploitation with strategies for safeguarding. This can be a great basis for building an individual education program to support someone to be sexually safe and have safe relationships. This assessment can take between 8 and 12 hours.
  • Assessment of Individual Skill
    An assessment undertaken by a Social Educator to determine areas that a person would like to work on, where they are at now, and where they would like to be.
  • Carer Strain Index

When a person sees us for an assessment, we can offer an associated personalised programs based on what a person would like to learn, and what areas were identified in the assessment process that could be improved.

NDIS Reports and Recommendations

Often the NDIA require clinical reports to provide evidence and recommendation for the support a person requires.
This is where our Social workers and Counsellors can help. We can write clinical reports and recommendations as well as reviewing other clinical reports to make further recommendations.

We can use either Improved Daily Living or Improved Relationships to deliver this support.
