Create A Sense of Place provides Care Coordination for people with a Home Care Package over the age of 65yrs, and 50yrs if you are Aboriginal or Torres Straight Islander descent.

The HCP Program supports older people with complex needs to live independently in their own home and uses a consumer directed approach to make sure that the support suits your needs and goals.

What safeguards are in place for you?

• Aged Care Quality Standards define what good care looks like in Australia. They apply to all aged care services that receive funding from the Australian Government.

• Charter of Aged Care Rights describe the legislated rights that apply to all consumers of Government subsidised aged care services (regardless of the type of care and services they receive.)
