Developed following the Safeguarding Assessment, this tailored support plan provides practical guidance for support workers on how to work alongside the individual respectfully and effectively.
An in-depth assessment using visuals and guided questions to explore an individual’s understanding of sexuality, relationships, and safety. It includes a detailed clinical report with recommendations for education, capacity building, and risk mitigation. This is often used as the foundation for a personalised learning program to support sexual safety and healthy relationships.
Designed to identify and respond to behaviours that may present a risk to the person or others. This includes a review of vulnerability, mental health, environmental and situational triggers.
Developed in collaboration with the person, this plan offers strategies for managing overwhelm, distress, or crisis. It includes practical recommendations for support workers to provide consistent, responsive care.
Our Social Workers can prepare detailed clinical reports required by the NDIA, including evidence-based recommendations for funding and support. We can also review other allied health reports and provide supplementary recommendations as needed.
